Overview
What is Hexarelin?
Hexarelin (examorelin) is the most potent GH-releasing peptide available. It produces roughly twice the growth hormone pulse of equivalent GHRH doses and has 30 times the receptor affinity of GHRP-6. It was developed in Milan and studied extensively in Turin, but development was discontinued around 2005 for 'strategic reasons,' not safety.
The defining problem is tachyphylaxis: with continuous twice-daily dosing, the GH response drops 31% in the first week and 45% by 16 weeks. About 55% of the response is retained (it never stops working entirely), and the effect fully reverses 4 weeks after stopping. The key finding: intermittent dosing (intranasal or oral, taken for days not weeks) avoids desensitisation completely.
Hexarelin has a unique feature no other GHRP shares: human cardioprotective data. In 24 bypass surgery patients, IV hexarelin improved heart function within 10 minutes. Neither GHRH nor direct GH injection produced this effect, confirming it works through CD36, not growth hormone. Despite this, the community has largely moved to ipamorelin (cleaner, no desensitisation) for GH purposes.
Mechanism
How it works
💪Strongest GH pulse of any GHRP
30x higher receptor affinity than GHRP-6. Peak GH of ~55 ng/mL at 2 ug/kg IV. Synergistic with GHRH (supra-additive). The most powerful single GH-releasing injection available.
📉Stops working with continuous use
GH response drops 31% in week 1, 45% by week 16 with daily dosing. Fully reversible after 4 weeks off. Intermittent dosing (days, not weeks) avoids this entirely.
❤️Protects the heart via CD36
Uniquely binds CD36 (scavenger receptor), improving cardiac function independently of GH. In bypass patients, LVEF improved within 10 minutes. No other GHRP has this human cardiac data.
⚠️Not selective (cortisol + prolactin)
Like GHRP-6, hexarelin raises cortisol (comparable to CRH) and prolactin alongside GH. Ipamorelin does neither. This is why most people use ipamorelin instead.
Benefits
Why is Hexarelin so popular?
What studies and real-world reports show so far.
bolt Maximum GH pulse for short cycles
The community values hexarelin for short, intense GH bursts. One user gained 4-6 lbs while looking 'significantly slimmer' over 21 days. Recovery from heavy training was dramatically improved: 'I didn't even feel soreness after deadlifting close to my PR, which is just absurd.'
face Dramatic skin improvement
One of the more vivid community reports. After 17-21 days: 'The skin around my stomach feels like a baby's. It's so smooth.' Nail quality also improved noticeably. These cosmetic effects from GH elevation are consistent with other secretagogues but appear faster with hexarelin.
favorite Cardioprotective effects (unique)
In 24 bypass surgery patients, hexarelin improved ejection fraction and cardiac index within 10 minutes via CD36. In hypopituitary males with negligible GH, LVEF rose from 64% to 70.7%. This cardiac benefit is GH-independent and unique to hexarelin among GHRPs. No community user reports choosing it specifically for this, but the clinical data is compelling.
verified Not on FDA Category 2
Unlike ipamorelin and GHRP-6 (both Category 2), hexarelin was not placed on the restricted compounding list. This is a practical advantage for US sourcing. The reason for this omission is unclear.

Guide
Dosing & escalation
How Hexarelin dosing typically works, based on clinical protocols and community experience.
syringe
100mcg SC + 100mcg Mod-GRF, 1-2x daily
Standard protocol. Fasted (2-3 hours post-meal). Always pair with a GHRH analogue for synergistic effect. Morning dosing gives the strongest pulse.
schedule
2-4 week cycles maximum
Desensitisation begins in week 1 and accelerates. Limit to 2-4 weeks then switch to ipamorelin or take 4+ weeks off. The 'kickstart then switch' approach is the community standard.
refresh
Intermittent dosing avoids the problem
Intranasal for 8 days and oral for 15 days showed zero desensitisation in clinical studies. Short bursts with breaks may be smarter than continuous daily injection.

Safety & side effects
What clinical trials and community experience show. Side effects tend to be worst in the first few weeks and at higher doses.
trending_down Tachyphylaxis (the defining problem)
GH response drops 31% in week 1 and 45% by week 16 with continuous use. About 55% is retained (it never completely stops). Fully reversible after 4 weeks off. The community limits cycles to 2-4 weeks then switches to ipamorelin. Intermittent dosing (intranasal TID for 8 days) showed zero desensitisation.
warning Cortisol elevation
Comparable to a CRH injection at GH-releasing doses (~40% increase). Works through the AVP (vasopressin) pathway, not CRH. With chronic 16-week dosing, the cortisol response actually decreased over time. No clinical HPA axis impact was observed, but the short-term elevation is why ipamorelin is preferred.
warning Prolactin elevation
Modest, less than TRH-induced. Potential gynecomastia risk in sensitive males with extended use. Ipamorelin does not raise prolactin at any dose.
bedtime_off Initial sleep disruption
Community reports: sleep actually worsened for the first 1-5 days before improving. By day 5-12, deep sleep improved significantly. This initial worsening phase doesn't occur with ipamorelin.
trending_down
Tachyphylaxis limits cycle length
GH response drops 31% in week 1 and 45% by week 16 with continuous use. Limit cycles to 2-4 weeks. Take 4+ weeks off for full recovery. Intermittent dosing (days, not weeks) avoids desensitisation entirely.
warning
Raises cortisol and prolactin
Cortisol elevation comparable to CRH (~40% increase). Prolactin elevation modest but present. Neither occurs with ipamorelin. The cortisol response actually decreases with chronic use (no clinical HPA impact observed), but short-term elevation is a concern.

Hexarelin has more clinical data than most GHRPs: tachyphylaxis quantified over 16 weeks, cardiac benefit demonstrated in bypass surgery, dose-response across multiple routes. Development was discontinued for strategic reasons, not safety. Evidence grade: LOW-MODERATE. Tachyphylaxis (quantified) +
The most thorough desensitisation data for any GHRP. Rahim and Shalet followed 12 subjects receiving hexarelin SC twice daily for 16 weeks. GH AUC dropped from 19.1 ug/L/h at baseline to 13.1 at week 1 (-31%), 12.3 at week 4 (-36%), and 10.5 at week 16 (-45%). The response was never abolished: 55% was retained at 16 weeks. After 4 weeks off, response returned to baseline. Separately, intermittent intranasal dosing (1.25 mg TID for 8 days) showed no desensitisation at all.

The strongest evidence for any GHRP in human cardiac function. Broglio et al. studied 24 coronary artery disease patients during bypass surgery in a controlled 4-arm design (hexarelin vs GHRH vs recombinant GH vs placebo). IV hexarelin (2 ug/kg) improved left ventricular ejection fraction (P<0.001) and cardiac index (P<0.001) within 10 minutes, lasting 90 minutes. Neither GHRH nor GH injection produced these effects, confirming the mechanism is GH-independent (CD36 receptor). In hypopituitary males with negligible GH release, LVEF still rose from 64% to 70.7%.

Half-life was never formally determined. No data beyond 16 weeks of human exposure. Why tachyphylaxis affects GH release but apparently not cardiac effects is unexplained. Whether intermittent dosing preserves both GH and cardiac benefits long-term is untested. No Phase 3 trials were ever conducted. Whether a CD36-selective analogue (without GHS-R1a activity) could be developed for cardiac-only use remains unexplored.

Frequently asked
Common questions
Why does hexarelin stop working?
Tachyphylaxis. With continuous twice-daily dosing, the GHS-R1a receptor desensitises. GH response drops 31% in week 1 and 45% by week 16. The receptor shows rapid calcium response fading within 2-5 minutes in vitro. About 55% of the response is retained (it never stops completely). The effect fully reverses 4 weeks after cessation. Intermittent dosing (intranasal for 8 days, or oral for 15 days) showed zero desensitisation.
Is hexarelin better than ipamorelin?
For short-term maximum GH output, yes. For everything else, no. Hexarelin produces roughly 2x the GH pulse but raises cortisol and prolactin, and desensitises within weeks. Ipamorelin is selective (GH only, no cortisol/prolactin), doesn't desensitise, and can be used indefinitely. The community consensus: use ipamorelin unless you specifically need maximum short-term potency.
What about the heart protection?
Hexarelin has unique human cardiac data. In 24 bypass surgery patients, IV hexarelin improved ejection fraction and cardiac index within 10 minutes. Neither GHRH nor direct GH injection produced this effect. It works through CD36, independently of growth hormone. In hypopituitary males with negligible GH response, heart function still improved. No other GHRP has this data. But no one in the community currently uses hexarelin primarily for cardiac benefit.
How should I cycle hexarelin?
Short cycles only: 2-4 weeks of daily dosing, then switch to ipamorelin or take 4+ weeks off. The 'kickstart' protocol uses hexarelin in the morning for maximum pulse and ipamorelin at other times. Intermittent intranasal dosing (a few days at a time) avoids desensitisation entirely and may be the smarter approach, but the community mostly uses subcutaneous injection.
