Injury Review Process - Berkeley Lab EHS

LBNL work related injury and illness reviews are conducted under the direction of the Environment, Health, and Safety Division’s Injury Review Program (see PUB 3000, Ch. 5.1). Injury review is directed by the Injury Review Program Manager. Injury review is a collaborative effort on the part of the Injury Review Program; Health Services; and division line management (typically through the division Safety Coordinator), work lead, employee, and subject matter experts. Each review is tailored to the nature of an event and is responsive to the individual safety management needs of involved divisions. Injury review does not look for or assign fault. The intent is to determine how injuries will be prevented in the future. An injury review consists of: Investigation Analysis of data Identification of causal factors Corrective actions Report Investigation timeline. Context first

Study findings do not automatically establish clinical benefit, safety, dosing, or suitability for an individual.

Investigation

Factual information about equipment, materials, location, conditions and personnel is harvested under neutral perspective.

Analysis of Data

The information is processed and analyzed to determine what caused the injury or illness. Some investigators are trained in the application of specialized causal analysis techniques and will be assigned to an investigation when this is required. Further information on causal analysis at LBNL can be found in PUB 5519-2.

ISM Analysis

Maximum improvement in LBNL systems and work performance is achieved when the root causes of an injury are related to weaknesses in the integration of safety management with the work performed. The ISM analysis relates the causal factors to improvements that may be made in management of our work at the institutional and division levels.

Corrective Actions

Corrective actions are proposed that will be effective in preventing or reducing the likelihood or recurrence of the injury.

Investigation Report

A report is prepared to document and communicate the investigation information, findings and corrective actions.
